Online Exemption Form
Absence from weekly training
Exemption requests DO NOT need to be submitted for a one-off absence from weekly training sessions. In this case, please contact your coach and advise them you are unable to attend training together with the reason(s) why. Players who are unable to attend weekly training sessions for a period of more than two (2) weeks, must request and exemption using the online form.
Exemption requests
Performance Squad Players or their parent/guardian can request an exemption to participate in a mandatory event or activity as part of the BV State Squad Performance Program, by completing the online form.
Supporting documentation for exemption
If you have any supporting documentation as part of the exemption request (e.g. a medical certificate) then please email it to performance@badmintonvic.com.au after submitting the application online.
